So it was the R2 or L2 trigger constantly giving signal. Can easily see this in windows.
Of course there is no Test App on the PS5.
I mean there should be. But there isn't.
I messed around with the ribbon controls after dissassembly and not it works fine.
I marked it with Purple Zero on back.
I got two controllers so I can swap easily when one battery dies.
Was working fine until say the past month.
Both start PS5 and work inside PS5 UI, but in Elden Ring or Dark Souls Remastered only one controller ever works.
For example. I turn On PS5 with Controller 2 but when it says Press Button to start in Game nothing happens.
I have to turn on PS5 with Controller 1 (Both are assigned to Player 1 BTW).

I have two controllers fully charged.
Controller 1 dies while playing,
How do I use controller 2 to coninue playing.
Controller assist does NOTHING. and doesn't work.
To continue playing with Controller 2 after Controller 1 dies, you can do the following:
Connect Controller 2 to the PS5 using a USB-C cable.
Press the PS button on Controller 2 to turn it on.
The PS5 will automatically recognize Controller 2 and assign it to the next available player slot.
Once Controller 2 is assigned to a player slot, you can disconnect the USB-C cable and continue playing with Controller 2 wirelessly.

If the PS5 does not automatically recognize Controller 2, you can manually assign it to a player slot by following these steps:
Go to the PS5 Control Center.
Select the Gear icon to open the Settings menu.
Select Accessories.
Select Controllers.
Select Assign Controllers.
Press the PS button on Controller 2.
Controller 2 will be assigned to the next available player slot.
Once Controller 2 is assigned to a player slot, you can disconnect the USB-C cable and continue playing with Controller 2 wirelessly.

If you are still having trouble connecting Controller 2 to the PS5, you can try the following:
Restart the PS5.
Try using a different USB-C cable.
Try connecting Controller 2 to a different USB-C port on the PS5.
If you are using a wireless controller adapter, try connecting the adapter to a different USB port on the PS5.

The 2nd controller connects fines and navigates the playstion UI from home just fine. It NEVER works in any game though. I understand all my games are one player. But how do I use the 2nd controller for a 1 player game?

To use the 2nd controller for a 1 player game on PS5, you can use the Use Second Controller for Assistance feature. This feature allows you to use two controllers as if they were a single controller, allowing you to control the same character in the game.

To enable the Use Second Controller for Assistance feature:
Go to Settings > Accessibility > Controllers.
Select Use Second Controller for Assistance.
Press the PS button on the 2nd controller to connect it to the PS5.
Once the 2nd controller is connected, you can use it to control the same character in the game as the 1st controller. Note that the 2nd controller will not have haptic feedback or adaptive triggers.
This feature is only available on certain PS5 games. To check if the game you are playing supports this feature, go to the game's options menu and look for a setting called Use Second Controller for Assistance.

Yeah that does nothing. So having a 2nd controller for when the 1st controller battery dies is pointless. Right?

In this case, I recommend contacting PlayStation customer support directly.
