As part of the appraisal process, EOs use the EPMS work review forms to document their contributions, their areas of improvement and training plans for future development purposes.
Completion of EPMS forms
The EPMS work review form consists of 2 sections:
Work Review Form
(Open Section)
Records your contributions, work plans and training plans
Allows Reporting Officer (RO) to guide you on possible areas for improvement
Completed by you together with your RO
Endorsed by the Countersigning Officer (CO)

Work Review Form
(Closed Section)
Gives an assessment of your overall performance and long-term potential
Serves as a summative record of your appraisal as endorsed by the school / division recommending panel
Prepared by RO; and routed to the CO
