CAse Referral Process
Case Referral Process
is primarily a 3-Stages process that guides teachers on the personnel within the school they can go to for advice and support. It also spells out the roles of different personnel at each stage and the types of collaboration to support pupils with additional needs.
Stage 1:
EMERGING Needs
Initial concern with difficulties of a pupil raised by the teacher and/or parent
Actions of support to taken by the teacher to address the problem
To complete Level 1 Intervention Form and Observation Form
Stage 2:
Mild to Moderate Needs
Difficulties of a pupil persists despite appropriate and sustained teacher intervention at Stage 1
Actions of support to taken by the teacher and Year Head to address the problem
May involve specialised resource personnel within the school, such as Discipline Team, Learning Support Coordinator, etc
Stage 3:
HIGH Needs
Difficulties of a pupil persists or worsens despite involvement of Stage 2 personnel OR when the case is perceived as high risk
Involvement of other specialised resource personnel, namely the Discipline Master (DM), SENOs, School Counsellor or direct involvement of external agencies
pupil intervention
Level 1 intervention form: for teachers
Before Level 2 Intervention are provided to the pupil by YH/SENO, the Form/Subject Teacher are encouraged to implement the suggested strategies and document the process and impacts. Information gathered are useful and can be used as evidence for assessment referral.
observation form: for teachers
Recommended to be used with the Level 1 Intervention Form, this enables the Form/Subject Teacher to provide more details to the pupil's area of concern(s). Guiding prompts provided can be expanded.
Counselling
Disclaimer: If pupil has signs of self-harm or suicide, please alert the YH and School Counsellor immediately. Risk assessment will be done without the submission of forms.
case Information Gathering form: for teachers
For the School Counsellor to have a better understanding and ease of communication with the pupil, Form Teacher needs to complete the CIG Form.
consent form: for parents
Before the School Counsellor begins the counselling sessions with the pupil, parental consent must be obtained.
