Commentary/Procedural Points:
The Department of Human Resources has designed the Archdiocese of Baltimore's Employment Reference Form to use for references from the applicant's supervisors. This form is used for all school staff, such as teachers, instructional assistants, guidance counselors, administrative staff, etc.
The State of Maryland has designed the Maryland's Employment History Review Form: (Child Sexual Abuse and Sexual Misconduct Form) for each employer where the applicant has worked (entire work history).
The Principal (Responsible Administrator) must keep records of efforts to obtain references even if these efforts do not result in direct contact and response.
Sample forms for checking references are provided in the Procedures for Implementation and Compliance: A Statement of Policy for the Protection of Children and Youth on the Archdiocesan website.
