Step 1 – Live Scan
Before you are able to complete and mail out the application, you need to get a live scan done. You can schedule an appointment for your live scan by going to IdentoGo. (Or you can call (877)472-6915). When completing the online application, be sure to note the following:
1. Select Digital Fingerprinting and enter the service code: 1545TY to begin enrollment.
2. Enter your personal information and choose a location and time for your appointment.
*Once done, be sure to save the receipt as it must be submitted with your Employee Statement form.
Step 2 – Form
You need to print the Employee Statement form and complete it entirely, answering every question honestly. Do not forget to sign the bottom of page 2.
*You MUST include 3 years of employment history or your application will not be accepted.
**You are required to include court dispositions of any charges you list on the form.
Step 3 – Submission
Once the form is complete and the live scan done, you need to send both to:

A: A new application can take up to 1 month to process once everything is received by the state.
Q: Once issued, how long is the license good for?
A: This registration does not expire, it is valid until removed as an employee.
Q: Do I need to switch my registration from one company to another?
A: You cannot switch your registration from one company to another, you will need to reapply and get a new live scan.
