Check-In (Front Desk)
Review the patient’s status from the appointment screen.
Verify patient information and confirm any updates, including insurance details and contact information.
Ensure that all necessary forms and consent documents are completed.
Rooming the Patient (ATC/MA)
History of Present Illness (HPI): Gather detailed information regarding the patient's current condition and concerns.
Vitals: Record the patient's vital signs (e.g., blood pressure, temperature, pulse, respiratory rate).
Physical Exam: Conduct a thorough physical examination, noting any relevant findings.
Kiosk Completion: Ensure that the patient has completed the necessary kiosk forms (e.g., health history, current medications, allergies).
Imaging (Xray Tech)
If applicable, Xray tech will escort the patient to the imaging area or confirm imaging orders.
Verify that the correct imaging procedure is scheduled and assist the patient as needed.
See the Physician or Physician Assistant (PA)
Once the patient is ready, ensure they are seen by the physician or PA for further evaluation and treatment recommendations.
Provide the physician/PA with a summary of the patient’s HPI, vitals, and exam findings.
Close Out Appointment (Team)
Document any changes to the patient’s treatment plan, prescriptions, or follow-up care.
Provide any necessary instructions or educational materials to the patient regarding their condition or care plan.
Check-Out (Front Desk)
Ensure the patient schedules any follow-up appointments or procedures.
Provide any necessary paperwork, such as referral forms or insurance claims.
Review the patient’s next steps and answer any remaining questions they may have.
By following these steps, we ensure a smooth, organized, and professional experience for each patient.
If a patient brings outside imaging or medical records to their appointment, please ensure that these are uploaded promptly or handed directly to the team before the patient is escorted back to the examination room.
To optimize the workflow, it is highly beneficial to upload these materials prior to the patient being placed in the room. This allows the team to review all relevant documents while the patient is completing their check-in process at the kiosk.
