Reason for Cancellation:
When canceling an appointment, ensure that the reason for the cancellation is noted in the system.
Example format:
Year/Month/Day - Patient cancelled due to being sick, [Your Initials].
Reminder Call:
Patients will receive a reminder call for their scheduled appointments approximately 48 hours in advance.
Late Cancellations & No-Shows:
Non-attendance or Cancellation without 24-hour Notice:
A $50.00 fee may be charged to the patient’s account to cover administrative costs and time reserved for the appointment slot.
EMG or MRI Appointment:
Failure to attend or cancel an EMG or MRI appointment without a 24-hour notice may result in a $200.00 fee charged to the patient’s account.
These fees are at the doctor’s discretion and are administered by the billing department.
Interpretation Services:
Cancellation/Rescheduling Notice:
If the patient requires interpretation services, we require a 72-hour notice for cancellation or rescheduling.
Failure to Provide 72-Hour Notice:
The patient will be responsible for the cost of interpretation services and any travel costs if they fail to notify us at least 72 hours in advance.
Patient Acknowledgment:
The patient’s signature on the agreement indicates their acknowledgment that they will be billed for interpretation services and travel costs if they cancel or reschedule without proper notice.

Action:
Email Referrals Team to notify them of the cancellation, including who was cancelled and when.
When a patient calls to cancel an appointment with an interpreter scheduled:
Email VVI for Eagle County or Call Erick/Laura for Summit County.
Notate in the general field that VVI or Erick/Laura has been notified.
Important Notes on Interpretation Services:
A 72-hour cancellation/reschedule notice is required for interpretation services, as they need advance notice.
Patients sign an agreement acknowledging they will be billed for interpretation services and travel costs if they fail to notify us at least 72 hours before the scheduled visit.
