Appointment Cancellation (Cx) Procedure:

Cancel/Re-Schedule Appointment:
When canceling an appointment, ensure that the reason for the cancellation is noted in the system.
Example format:
Year/Month/Day - Patient cancelled due to being sick, [Your Initials].

If Re-Scheduling, please provide date of new appointment 

 Late Cancel/No Show:

•  Non-attendance or Cancellation without 24-hour Notice:
  A $50.00 fee may be charged to the patient’s account to cover administrative costs and time reserved for the appointment slot.

*These fees are at the doctor’s discretion and are administered by the billing department.

Cancel Surgical/Injection Procedure:

FD/CC DOES NOT CANCEL SURGICAL APPOINTMENTS or EMG

Cancel Interpretation Services:

• Cancellation/Rescheduling Notice:
  If the patient requires interpretation services, we require a 72-hour notice for cancellation or rescheduling

•  Failure to Provide 72-Hour Notice:
  The patient will be responsible for the cost of interpretation services and any travel costs if they fail to notify us at least 72 hours in advance.

•   Patient Acknowledgment:
  The patient’s signature on the agreement indicates their acknowledgment that they will be billed for interpretation services and travel costs if they cancel or reschedule without proper notice.

** Personal scheduling procedure is responsible for communicating any changes to appointment to interpretation Service**

Canceling EMG or MRI Appointment:
**Failure to attend or cancel an EMG or MRI appointment without a 24-hour notice may result in a $200.00 fee charged to the patient’s account. **

EMG - FD/CC DOES NOT CANCEL EMG APPOINTMENTS -Transfer all calls to Surgery Schedulers x 6551 MRI -  FD/CC can cancel the appointment and send a message to the MRI team chat.
