Copay: A fixed fee that is required by contract with the insurance company to be collected at the time of service.
Co-Insurance: For a given insurance policy, the coinsurance is the percentage of costs the insured is responsible for paying once the deductible has been met. The payment of a co-insurance is not able to be waived; it is a contract between the patient and the insurance company to offset the risk of overutilization.
Deductible: The amount for which the insured is responsible before health benefits begin. The payment of deductible cannot be waived it is a contract between the patient and the insurance company.
Responsible Party: Whomever brings a minor patient in for services is responsible for payment of any co-pay, deductible or co-insurance regardless of what a custody arrangement says. We are not to get in the middle of who pays what and when. Payment is due at time of service. (Not always the subscriber)
Subscriber: Primary policyholder on the insurance, will need Name, DOB of subscriber if different from self
