ECH Patient Booking Guidelines – 

At Time of Booking:

• Collect payment and referral – Scan both into the patient chart.

• Do NOT need a self-pay waiver.

• Discount is applied to billed charges, not the self-pay rate.

Referral & Visit Limits:

• VSO accepts 4 new ECH patients/month.

  • Of these, 2 can be Medicaid (copay collected at visit).

• All appointments require:

  • Written referral from ECH

  • In-person scheduling

• Without referral = regular self-pay or insurance patient

Restrictions:

• No new spine patients

• No VSO MRIs – must be done at St. Anthony Summit

• No walk-in X-rays – X-rays only with scheduled visits

• If surgery is needed, patient must arrange payment with the surgery center and anesthesia

Other Notes:

• ECH assigns fee codes for discounted services (excludes DME, meds, casting).

• ECH patients must pay full remaining balance at time of visit.

• Patients can be seen in Eagle or Summit locations.

  • If patient lives in Summit, Eagle, Park, or Lake County, notify Frisco Front Desk (they track ECH patients).
