Second Opinion Protocol
Purpose:
This protocol ensures that all necessary steps are taken when a patient requests a second opinion for a medical issue. The goal is to gather the appropriate information and medical records to facilitate a comprehensive evaluation and decision-making process.
Questions to Ask the Patient:
Is the appointment for a referral or a second opinion?
Have you been previously seen for this issue?
Did you undergo surgery for this issue?
If yes, when and where was the surgery performed?
Who was the surgeon?
What imaging has been done related to this issue?
Please provide details on the date, location, and type of imaging (e.g., X-ray, MRI, CT scan).
Request reports for all imaging performed.
For acute issues:
Were you seen in an urgent care (UC) facility or emergency room (ER)? If yes, gather relevant details.
What to Document:
Appointment Scheduling:
If possible, schedule the appointment far enough in advance to allow adequate time for obtaining the required records.
General Notes:
Document all collected information in the General Notes section.
Confirm that the patient understands and agrees to bring all required documentation for the second opinion to their appointment.
Phone Note:
Send a phone note to the appropriate team members with the detailed information provided by the patient. This ensures all parties are informed of the patient’s request and the necessary follow-up actions.
Patient Follow-up:
Advise the patient to call prior to their appointment to verify that we have received all necessary medical records and documents for their second opinion.
Objective:
By following this protocol, we ensure that all relevant information is gathered and communicated effectively, facilitating a smooth process for providing the second opinion.
Referral Process: If patients inquire about orthotics, Dr. Elton refers them to Xtreme Footwerks for further evaluation and fitting.
