Social Security Numbers are REQUIRED for all self-pay patients, unless they are international.
OFFICE VISIT:
A DEPOSIT OF $417.00 is required at check-in and will be applied toward the total cost of the visit. When asked for an estimate, please quote between $550-$800, as the final cost may vary depending on whether the provider orders X-rays, DME, injections, or other treatments during the visit.

At Check-In:
Collect the $417.00 deposit (applied toward total charges).
Let the patient know:
"The total cost is estimated between $550 and $800. Additional charges may be incurred depending on what the provider orders during the visit (e.g., X-rays, DME, injections, etc.). We are unable to determine these charges ahead of time."
Have the patient sign the Prompt Pay Waiver.
Scan the signed waiver into the chart.
Run the Prompt Pay letter and give a copy to the patient.
Send the Prompt Pay Waiver and EBO (Estimate Based on Order) to Billing, and include it with the EBO report.
At Check-Out:
Review the Charge Details to determine what the patient owes.
