Accommodation Proposal
Introduction
As an accepted candidate in the Nursing Program at Southwest Wisconsin Technical College, it is my goal to earn an associate degree in nursing and become a nurse in one of the local hospitals. However, to achieve my goal, I will need some accommodations because of my disability.
Disability
I am hearing impaired. I have no hearing in my right ear, but I have normal hearing in my left ear. I hear normal conversation fine within a distance of 10 ft and in a quiet environment but have problems hearing people in noisy environments and have no sense of direction of where sounds originate.
Accommodations Needed
Therefore, I am requesting the following accommodations. I am requesting preferential seating. I find if I sit close to the instructor the noise of the chairs moving, shuttling papers, and students talking do not bother me. In addition, I would like to request the use of an FM system to tape record the lectures in case the noise level gets higher than usual and I cannot hear the instructor even at a close distance or the instructor moves around the classroom when lecturing. I am concerned about hearing the pulse in the stethoscope having only one good ear. I believe if I had an amplified stethoscope this would not be a problem. Therefore, I would like to request the use of an amplified stethoscope.

Accommodation Proposal
Introduction
I am excited to be starting a career with Target as a new cashier. I started work on July 10th and have many strengths to offer the team, such as staying organized and working with people in a friendly and positive way. However, because of my disability, I will need some accommodations in order to reach my full potential.
Disability
I have a learning disability in reading. I have struggled to read at the same pace as my peers for as long as I can remember. This makes it harder for me to work at the same speed as others and still comprehend and remember everything I read. I have received special reading services in school since 1st grade and have learned some strategies and used accommodations that have helped me.
Accommodations Needed
Therefore, I am requesting the following accommodations. I am requesting additional time to complete any training that requires reading. In school, I typically received double time to complete tests and quizzes. This helped me to take my time reading and not feel rushed. When I can read at my own pace I can do well. I am also requesting as much printed material as possible. It is very difficult for me to track the words on a computer screen. On paper, I use a ruler or another piece of paper to read one line at a time. This really helps my brain stay focused on the words I’m supposed to be reading instead of wandering all over the page. I can read and understand better when I’m able to do this.

Accommodation Proposal for Postsecondary Education
Purpose: An accommodation proposal will help individuals with disabilities rationalize the accommodations they need. Sometimes it is a frightening experience meeting with the Disability Services Specialist or Support Services Specialist. Developing an accommodation proposal and sending it to the Disability Service Specialist or Support Services Specialist before meeting with him or her will make it easier for individuals with disabilities to defend their accommodation request.
Instructions: Develop an accommodation proposal that you will send to a Disability Service Specialist or Support Service Specialist of the college you are choosing to attend. The proposal should define your disability and the accommodations you need. Reasons should be given to justify the accommodation. Look at Taylor Adkom’s Accommodation Proposal or ask your case manager for another sample. As you write your proposal use the Accommodation Proposal Rubric as a guide. After completing the proposal, have two other students in the course read your proposal and complete the Accommodation Proposal Rubric below. Make revisions and add to your Summary of Performance (SoP) and add to your documentation information in whatever portfolio option you choose.

Workplace Skills and Accommodation Analysis
Purpose: To insure maximum performance on the job, it is important to know your abilities, challenges and the accommodations that will limit the challenges. In addition, it is necessary to have an understanding of the basic workplace skills and relate them to your abilities.
Instructions:
Review your disability summary accommodations from Learning Plan #2.
Read the Workplace Skills. Then use your disability summary and complete the chart below, putting a check mark in the correct column.
Choose 3 different careers of jobs based on your work in Learning Plan #3 and #4 and complete the Essential Functions.
Compare your check marks to the “essential functions” of a chosen career or job you are interested in applying for.
Decide: Can I perform the essential skills? Do I need to request accommodations?
