Part A: Documentation Checklist
Purpose: Having the appropriate documentation about your disability is very important to receiving the right accommodations. It is important to review and understand your Individual Educational Program, (IEP), Postsecondary Transition Plan (PTP) and Summary of Performance (SOP).
Instructions: Work with your teacher or case manager to locate these documents.

Part B: Read and Evaluate Documents
Purpose: Documentation of a disability includes vital documents that you should completely understand in order to ask for the right accommodations. It is also very important to understand how others are interpreting your disabilities. You need to realize that interpretation of your disability can change with different evaluators. Thoroughly reading disability documentation and asking questions about the documentation will give you a clearer understanding of their disabilities.
Instructions: Thoroughly read your Educational Evaluation and IEP/PTP or other documents chosen with your teacher or case manager and answer the questions below. You can refer back to your Vocabulary Definitions as you are reading these documents.
What terms didn’t you understand? Research and record definitions in your portfolio.
Who wrote your Educational Evaluation?
What questions do you have about your Educational Evaluation that you will ask your school psychologist?
What questions do you have about your IEP that you will ask your instructor?
What did you learn new about yourself after reading your Educational Evaluation and IEP?
Is there anything in these documents that you disagree with and, if so, what is it?
Name the individuals who were involved in your IEP process.
How was your voice heard at the IEP meeting? Consider a student led IEP.
Did you complete the WiTransition App and discuss the Postsecondary Transition Plan (PTP)?
