Purpose:  Accommodation documentation varies across postsecondary schools and for employment. It can be confusing and time-consuming to keep searching for what is needed.  It is important that individuals are prepared with the necessary documentation and information in order to fully complete and understand the process.  

Instructions:  Use the checklist below to plan what documentation you need and how you will keep it all together.
