Assignment 11.5—Agency Documentation Checklist

Purpose:  To receive agency services, students with disabilities may have to complete many forms and submit documentation of their disabilities.  Not having all the correct forms and documentation may result in not getting services in a timely manner or not receiving services at all. Making a checklist of all the forms that need to be completed and documentation to be sent will help to be organized.  Complete forms and submit them to agencies as soon as possible.
