Lewis was drafted from the Hunterville Mounted Rifles and attested on 3 January 1902 at Trentham, Upper Hutt (Wellington). He was aged 20 years and recorded as being 5', 10" tall with a fresh complexion, dark hair and grey-brown eyes. His address was Hunterville, Rangitikei. His next of kin was Samuel Weston (father), Hunterville, Wellington.
