CONTEXT
The Patient-Doctor Relations (PDR) course is designed to develop effective communication, interviewing skills, and a humanistic approach in students as they interact with simulated patients and extract comprehensive clinical histories. It also offers opportunities to learn and demonstrate basic clinical skills such as medical handwashing and the measurement of vital signs. At the end of the course, the following outcomes were achieved: demonstrate clinical competence (PO1), effective communication (PO2), and engaging in personal and professional development (PO7).
EXPERIENCE
Throughout the PDR course, I participated in a variety of hands-on activities and simulations, each contributing uniquely to my understanding and skill set in clinical practice. The modules on medical handwashing and vital signs, interviewing techniques, adult medical history taking, pediatric history taking, and OB-GYN history taking were all impactful for me, as I had no prior experience with these in my pre-med studies. While I found the vital signs module to be the most enjoyable, history taking proved to be the most challenging for me.
First, the hands-on practice sessions were invaluable. I performed medical handwashing, measured vital signs, and did history taking repeatedly which significantly enhanced my clinical skills (PO1). These sessions also allowed me to apply theoretical knowledge practically, further solidifying my clinical competence. Second, as we performed these tasks, clear and precise communication was essential in every procedure, both for instructing patients and for documenting information accurately, thus fostering effective communication skills (PO2). Particularly in the history taking, simulated patient encounters and practice sessions highlighted the significance of empathy and clarity in communication, especially when dealing with sensitive topics. Lastly, everything that has been taught to me by my teachers and facilitators along with all the feedback I have received on my mastery and performance of the procedures have ultimately shaped me not only academically but also professionally (PO7). I am no longer the same person that I was when I started this course, and I am confident that I now have the skills and attitude that will help me become a more competent physician in the future.
REFLECTION
My experience in the PDR course led to several learnings. First, I came to understand that clinical competence extends beyond performing tasks well (PO1). It involves understanding the rationale behind each step and recognizing its implications for patient care. This deeper understanding is crucial for delivering high-quality care. Second, I learned that communication in healthcare is multifaceted, encompassing verbal and non-verbal cues, active listening, and the ability to convey complex information in an understandable manner (PO2). Additionally, clear instructions and empathetic interactions significantly impact patient compliance and the accuracy of extracted histories. Lastly, I realized how essential continuous self-improvement and a commitment to lifelong learning are in the medical field (PO7). Being able to embrace feedback as a tool for growth, rather than a critique of ability, helps foster resilience and personal development.
ACTION
First, in order to continuously enhance my clinical skills, I will continue practicing medical handwashing, vital signs measurement, and history taking at home, using checklists to evaluate my performance. Volunteering for medical activities and simulations can also provide additional practical experience for me. Second, to improve my communication skills, I plan to participate in role-playing exercises and communication workshops. These will help refine my ability to convey information empathetically and clearly. Seeking feedback from mentors and peers can also provide valuable insights into areas for improvement. Lastly, I strive to stay committed to personal growth and continuous learning. I plan on engaging in more extracurricular activities that build social skills and empathy, such as community service, as these will not only enhance my interpersonal skills but also help me grow in my professional demeanor.
EVALUATION
In conclusion, the PDR course has been a transformative learning experience for me. It provided me with a comprehensive foundation in both technical skills and interpersonal interactions. My initial uncertainties and fears, particularly in modules like history taking, have been replaced with confidence through dedicated practice and constructive feedback. The course has definitely equipped me with the knowledge and skills necessary to excel in my medical career. Moreover, the emphasis on patient-centered care and continuous self-improvement will guide me as I strive to provide high-quality healthcare with compassion and professionalism. Overall, I am grateful for the opportunities this course has provided and am committed to further developing my skills to become a competent and empathetic healthcare provider in the future.
