CONTEXT
This module tackles obstetrics and gynecology history taking. This involves gathering detailed information about a patient's reproductive health, including menstrual history, sexual history, obstetric history, gynecological symptoms, among others. This comprehensive approach helps healthcare providers diagnose and manage conditions related to female reproductive health. Effective OB-GYN history taking requires clear communication, systematic questioning, and a sensitive, patient-centered approach. Through this module, the following program outcomes were achieved: communicate effectively (PO2), use a systematic approach (PO6), and engage in personal development (PO7).
EXPERIENCE
This lesson was the most challenging for me given how extensive the procedure was as well as the incorporation of computations. However, despite my difficulties and initial confusions, the module was able to incorporate various activities that honed my abilities in history taking. First, standardized patient encounters, where we role-played scenarios with simulated patients, allowed me to practice effective communication (PO2). These encounters helped me learn to actively listen to patients' concerns, ask open-ended questions to elicit detailed information, and clarify any ambiguities to ensure a complete understanding of their medical history. With the help of the standardized procedure and sample history that was given to us, I was also able to use a systematic approach during my history taking (PO6). This method ensured that I documented all relevant patient information in an organized and concise manner. In addition, the feedback sessions from our facilitators also helped me determine my weaknesses and strengths (PO7). Reviewing the history taking techniques and taking note of the constructive feedback allowed me to identify areas for improvement in my history taking approach.
REFLECTION
First, I acknowledge how valuable the practice patient encounters were in developing effective communication skills (PO2). Initially, I found it challenging to balance active listening with asking probing questions to obtain more details. However, with practice, I learned to navigate the conversation, ensuring I captured the patient's perspective while gathering necessary clinical information. The importance of using appropriate medical terminology while remaining sensitive to the patient's comfort level and knowledge was also something I learned. Second, I realized that using the standardized procedure given to us and history writing exercises helped solidify my understanding of the importance of using a systematic approach in OB-GYN history-taking (PO6). The steps provided a clear structure to organize the patient's history, ensuring that I covered all essential elements of an OB-GYN history, minimizing the risk of missing crucial details. Lastly, I learned the importance of having a growth mindset in my personal development and commitment to lifelong learning (PO7). By taking feedback as constructive instead of attacks towards my abilities, I gained valuable insights into my strengths and weaknesses. Moreover, identifying areas where I could improve such as my communication style or the comprehensiveness of my questioning techniques motivated me to actively seek further learning resources.
ACTION
First, to further enhance my communication skills, I plan to consult my facilitators who are experienced OB-GYNs. Learning how they interact with patients will provide valuable insights into effective communication techniques in a real-world clinical setting. Additionally, I will practice active listening skills through role-playing exercises that focus on active listening before integrating them with question-asking in future encounters. Second, to truly master the systematic approach of OB-GYN history taking, I will develop a personalized checklist, outlining key areas to cover during a history to ensure I consistently gather all relevant information. Lastly, my commitment to personal development includes learning more from my weaknesses and taking more proactive steps towards improving them. For instance, participating in communication skills workshops or extracurricular activities that help build my social skills and empathy can help me gain the confidence to engage in better and more effective patient encounters in the future.
EVALUATION
In conclusion, this OB-GYN history taking module significantly contributed to my growth as a future healthcare provider. The integration of standardized patient encounters, history writing exercises, and feedback sessions provided a comprehensive learning experience for me. As a result, I was able to learn from my mistakes and identify areas for improvement such as my mastery of the steps and computations. Overall, this module was instrumental in developing my communication skills, systematic approach, and commitment to personal and professional development. Because of this, I know that I am better equipped with the knowledge and skills to excel in my medical career and provide high-quality patient care in the future.
