Teacher_Summer Check Out & Campus Forms

Instructions

Whether you are leaving the district or not, you must complete the summer checkout forms.

1. Please notify all maintenance folks & aides that they DO NOT unplug or move technology, but are to notify the Tech Dept when they are ready for us to move teacher stations and labs, etc.

2. Make sure you have on file these forms CIPA Form (Teachers Only) & LOTI Form (Teachers Only for beginning and end of year)

3. Campus Checkout Form

• Campus End of Year Form: Download & Print this Form to get signatures from all the respective folks, whether or NOT you are leaving.
