Normalise failure. This means destigmatizing it. Describe what the value of failure means to the system. It's a critical part of the learning cycle.
Air it out. Italian saying ' the fish on top of the table stinks less than one under it'. Put your failures (fish) on top of the proverbial table, and let the air circulate.
