We attribute success to our own efforts (internal) and failure to external events (or others).
Normalise failure. This means destigmatizing it. Describe what the value of failure means to the system. It's a critical part of the learning cycle.
Air it out. Italian saying ' the fish on top of the table stinks less than one under it'. Put your failures (fish) on top of the proverbial table, and let the air circulate.
