Log into Ideal-Logic
Click on Staff Home on the Left Hand Side
Click on Incident Accident/Behavior Report
Click the Appropriate Report type. If it was a behavior report but there was an accident involved, click behavior report, and it will include questions pertaining to the accident report.
Behavior Report
Search the name of ALL participants involved in the incident and add them to the report.
Add any involved adult participants. This would only apply to a situation where maybe a guardian was also involved in the behavior incident, or involved in the administration of first aid if it was required. This will not get used very often. This does NOT apply to anyone on staff (including CITs - included as staff in this case, or TLs - included as participants).
Add all staff members that were present. This would include any staff that witnessed the incident, helped administer first aid or were with the group.
Add any witnesses to the report - this would be like if a leadership staff was helping you complete the report in a more serious injury situation, but they were not directly involved in the incident.
Answer all of the questions under the questions tab. For the free response sections, ONLY THIRD PERSON CAN BE USED!!!!
Example: Jack threw a marker at Jill. Kiley asked Jack not to throw the marker, and then Jack threw another marker at Kiley.
Determine if any injury did or did not occur. If an injury did occur:
Put a detailed description of the entire behavior incident, referencing when the injury occurred. Include all participants and staff involved.
Under detailed description is where you put a detailed explanation of how the injury occured
Please describe the care given is where you explain exactly how care was administered
If an injury did not occur, put a detailed description of the entire behavior incident, including all staff and participants involved.
Then, answer the remaining questions:
Participant follow-up is what you did afterwards. Include all follow up in regards to both the injury and the behavior incident (ex: after Jack fell, Kiley took Jack to the office and informed Janet of the incident. Kiley, Karen, and Jack all worked to complete a contract for how Jack would behave appropriately the next time Jack was in art class).
Parent/Guardian follow-up is where you explain exactly what happened with the Parent/Guardian. (ex Kiley spoke to Jack’s mom, Janet Smith, at checkout, and told Janet about the incident. Janet told Kiley thank you for telling her [Janet], and that Janet and Jack would have a conversation at home tonight about the appropriate way to behave in art class, and come back to camp tomorrow with a behavior plan for what to do if Jack is not behaving in art class)
Include the full name of the guardian you spoke with and their relation to the camper
Include when you talked to them
Include their response and any additional follow-up that was required.
The parent guardian follow-up can also include leaving a voicemail (should denote that a voicemail was left, or if a leadership staff is going to speak with the guardian then that should be indicated in the report, and you should slack the appropriate leadership staff and ask them to fill out their section of the report after speaking with the guardian).
Behavior incidents often involve leadership staff. The witnessing/ first intervening staff should always complete the report, and have leadership staff add to it. Typically leadership staff should be informed of behavior incidents and are the ones speaking with the guardian, especially if it is a recurring behavior issue, or a camper who’s guardians we are working with more closely.
