Protocol for Employee (On the Job) Injuries
** CONTACT RISK MANAGEMENT FOR WORKERS' COMPENSATION CLAIM PACKET **
The injured employee should complete, in its entirety, the (recently updated) Employee Report of Injury form. This form replaces the old WC‐1 injury report.
Both the employee and supervisor should sign the report, and a copy should be sent to the Risk Management Department within 48 hours.

A copy should be given to the employee and the school should keep the original for the site file.
The employee should review the Panel of Physicians and select one of the clinic for medical attention. Some clinics operate 24/7 for after hour needs. Should there be a need for medical attention, the initial visit should take place within 8 hours of the injury.
Per Board Policy, drug and alcohol testing is mandatory following an on the job injury. Employees found in violation of this policy shall be terminated and are not eligible for reemployment.
Should an employee elect to seek treatment with their Primary Care Physician, it will not be covered under Workers' Compensation. An email or written note declining medical treatment under Workers' Compensation should be sent to the Risk Management Department (Attention: Melanie Mabry) within 48 hours.
Please direct the employee to choose one of the approved clinics from the Panel.
Only send to an Emergency Room in a serious situation and call the Risk Management Department immediately to notify of emergent care.
When an employee seeks treatment (initial or follow up) from a Workers' Compensation Physician, they are to clock out for timekeeping purposes and should be coded accordingly. Time used for medical treatment will be deducted from sick leave and should be coded (19) for Workers' Compensation.
Additional forms in the packet are as follows: Panel of Physicians, Bill of Rights for the Injured Worker, Medical Release (WC-207), Salary Election Form, Mileage and Pharmacy forms. These documents are for the injured employee’s records.
For all job-related injuries or illnesses that meet the criteria for a serious health condition under the Family and Medical Leave Act (FMLA), the absence for job- related illness or injury will run concurrently with FMLA.
It is the responsibility of the employee to stay in contact with their supervisor regarding work status and absence management following medical treatment, keeping them apprised of future appointments, absences and job limitations.
