When students are injured at school, student accident report should be completed by the teacher witnessing the accident/injury. This form is completed electronically and submited via email to school principal and front office clerk. Once submitted, the parents should be notified by the teacher as well as the nurse (if necessary). Notification should take the form of a phone call, or a note on the choice card if no one is able to be reached by phone. Please do not print, or send home copies of the accident report. Parents may request copies in the front office.
