1. Exact location of the patient 

2. Chief Complaint/Patient Information: 

A. What is the main problem with the patient?  

B. Is the patient conscious and alert?  

C. Is the patient breathing? Are they having any difficulty breathing?  

D. Male or Female 

E. Age / approx. age or DOB 

ACTION 

• Use UVMEDICAL as Spillman Nature code 

This will create a Law case for UVU and Fire case for Orem Fire - remove the "F" type if Orem Medical is not needed. 

• Any Medical on campus that is NON-EMERGENT: 

• If you have the UVU ERT’s (28AEMS) on campus, reach them on the UVU Med channel on the radio. - If they are not on duty or not avail, you will need to have Metro send Orem Fire Department using the "F" type of the call. 

• Dispatch 28AEMS unit and an ON Duty Officer to the LAW call.  

• IF THE FOLLOWING APPLY – Metro will assign Orem Fire units to the “f” CALL FOR A RESPONSE (Metro will immediately send an ambulance using your call comments)  

1. Cardiac Arrest or Distress (Any chest pain) 

2. Cardiac History 

3. Respiratory Arrest or Distress 

4. Unresponsive Person or Unknown Problem 

5. Loss of Consciousness 

6. Altered Level of Consciousness 

7. Traumatic Injury
