There are numerous ways to acquire equipment.
Using the blacksmith in Evermore is the most common way to acquire equipment, as the base materials drop regularly from a wide variety of monsters. Forging 10 pieces of equipment is also a daily to-do task, so you'll be using it frequently regardless. Equipment is randomly rolled and is weighted towards 1* and 2*, but any equipment available in the summon shop can be forged here, including rare 4*. Additionally, after you forge a certain amount of equipment, a piece of equipment will be guaranteed to be 3* or 4*.
Talk to Tunba, the raven who runs the forge in the western area of Evermore. He can forge weapons and armor (the first option), boss accessories (the second option), or orbs (the third option.)
The first option will forge weapons (the left tab) or armor (the right tab). You can forge up to 100 pieces at a time as long as you have enough recipes, base materials, and gold.
Tapping on the magnifying glass icon on the left will show you the rates for each piece of equipment that can be forged. The second tab shows the rates for the next guaranteed roll.
You can see the results of the forging. All equipment is automatically added to your inventory. If your inventory is full, they will go into your inbox. You will need to clear out some space in your inventory before you can claim them out of there.
Tapping on the info box below Tonba will show you when you are guaranteed to roll a 3* or 4* equipment. 3* is guaranteed at 30 and 90, and 4* is guaranteed at 200. After the 200th, the counter is reset.
If you will hit the guaranteed roll with your current forge settings, you will get an info box asking you to confirm you wish to forge. Since weapons and armor are separate tabs, be sure to choose the type of equipment you want to forge for your guaranteed star level piece of equipment.
The second option will allow you to forge boss "sealed" accessories.
Each accessory costs 50,000 gold and 100 purified souls from that field boss. Purified souls are a common drop and reward for defeating field/world bosses, and you also earn some for completing a bounty quest against that field boss.
Sealed accessories must be rolled into their final form, with a 90% chance at a 3* accessory, a 9% chance at a 4* accessory, and a 1% chance at a rare 4* accessory.
During Field Boss Season Pass periods, an additional option for forging that season's boss accessories becomes available and require the usage of special drops from that field boss.
The third option will allow you to forge orbs.
Depending on what materials you have, you can obtain boxes which have a chance to roll orbs of differing ranks. The highest rank boxes will guarantee a 4* orb (with a low chance of rares), the middle rank boxes will roll between a 2* and 4* (with a very low chance of rares), and the common box will roll beween at 1* and 3*.
You can also forge a 4* rare directly with a normal 4* of the same element and type that has been awakened to level 10.
Every 20 levels, your class will get a reward. Some of these rewards include weapon boxes with weapons for that class.

4* weapon selection box: a box that allows you to select 1 normal 4* weapon (fire, water, or wood element)

Certain monsters can drop equipment.
Field/World bosses can drop sealed accessories directly when defeated, and the reward chests from them may also provide a sealed accessory. However, you are more likely to acquire field boss sealed accessories by forging them in Evermore with boss material drops.
Certain monsters in various areas of the Chaos Dungeons can drop weapon, armor, accessories, or orbs. A monster will only drop one type of equipment, but will drop them at base star level of 1* through 3*. These groupings are:
Weapons for all classes of the same element as the monster
Armor of all types
Earrings (normal)
Rings (normal)
Necklaces (normal)
Either attack or defense orbs of the same element as the monster
Each Dimension Border boss has a boss accessory that it can drop. These accessories are all 4* and their first skill boosts elemental damage for its associated element.
Natrum earrings (wood)
Proto Mk-XIII necklace (water)
Volcanord ring (fire)
Additionally, if you purchase the premium boundary pass for that boss, you will earn accessories at steps 5, 25, and 45.
Rarely, an event will be run that rewards equipment if the task is completed during the event period. Rewards can be for a specific piece of equipment or a selection box that either rolls one randomly or allows you to choose from a list. For example, the 100-days login event rewarded a 4* weapon/armor box on day 7 and a rare 4* weapon/armor box on day 14.
The shop has a few places where you can buy equipment.
Several packages in the shop include accessories. These are typically 3* normal accessories like the mighty necklace, so their worth is higher for newer players.
The best deal in the pack shop is the "free" (technically it costs 10 in-game gold) login pass. The reward on the 14th day is an equipment box that will randomly roll a normal 4* weapon or armor. Once this has been acquired, the pass resets and you can buy it again, meaning you can get 2 pieces of (random, normal) 4* equipment every month this way. A second daily login bonus costs 100,000 gold per 14 days but provides numerous low rank orb boxes as well as materials to upgrade orbs.
Currently, the equipment summon shop can be used to acquire weapons/armor or orbs. Unlike the familiar summon shop, there are no banners with increased rates, so you should feel free to roll tickets whenever you have a good batch saved up.

Tapping on the leftmost magnifying glass in the left will show you what the rates are for a category of equipment (such as 4*) as well as an individual piece of equipment. The second tab in this infobox shows the rates for the 100th and 200th roll, which guarantee a 4*.
The summon shop uses either tickets or diamonds to roll. Tickets and diamonds are typically acquired as rewards, from coupon codes, or from in-game purchases. Diamonds can also be gathered from the Familiar Adventure game mode.
The equipment summon shop has a step counter that increments by 1 per roll done with diamonds, or 0.5 per roll done with tickets. If you increment the counter to 100, you are guaranteed a 4* piece of equipment on the 100th step and 200th step.
Steps are counted separately for the weapons/armor shop and the orbs shop.
