Go to the fillable PDF from the OUSA insurance page.
Fill it out, with the following fields:
Club Name: Navigation Games
Club Contact: yourself, or most relevant person
Address: 116 Henry Street, Cambridge MA 02139
Generally, check Yes to the additional insured question.
Provide complete address for the additional insured. Typically, this is DCR for which you use the following information:
Name: The Commonwealth of Massachusetts, Department of Conservation and Recreation
Address: 10 Park Plaza, Suite 6620, Boston MA 02116
Provide the event information and your name. Note: Chrome seems to work but not Firefox for filling out the PDF form. Print to PDF instead of saving as PDF (the latter sometimes does not preserve the information).
From DCR: "You must submit a certificate of liability insurance (COI) at least 14 days before the event. The Commonwealth of Massachusetts, Department of Conservation and Recreation must be named as additional insured for the date and location of your event with a minimum amount of 1 million dollars ($1,000,000) in liability. This is required for all special use permits and your permit will not be issued without the certificate."
Once obtained, upload to DCR. To upload go to, https://eplace.eea.mass.gov/citizenaccess/ click "My Records" tab and find your application and click on the record number. Click on Record Info dropdown and select attachments to upload the insurance document.
Then, put in Administration > 07 Insurance > OUSA general liability > COIs we obtained from OUSA
Contracts and invoices sheet in program data - indicate that we got the COI
Note about the PDF:
I haven't been able to fill it out in Firefox. Chrome works. Adobe Reader DC might work too, but that program is slow for me.
From Chrome, I print to PDF instead of saving as PDF. The latter does not preserve the information that I typed in.
