Report workers in your facility who have been influenza vaccinated from October 1st through March 31st.
The reporting deadline is May 15th.
Sign into COHFI.
Select your facility.
Select Vaccination Reporting and then Flu Vaccination Reporting.
Complete the form.
Select Send Vaccination Report.
Select Vaccination Reporting from the top navigation bar and then select Flu Vaccination Reporting.
If you do not have Vaccination Reporting available then check with your facility/agency's account manager to allow you access.
Complete the form by answering each question with the number of Employees, Licensed independent practitioners, and Adult students & Trainees.
The reporting period is October 1st of the previous year through March 31st of the current year.
The reporting deadline is May 15.
To be counted, employees must work at least one day.
Select Send Vaccination Report to submit your report.
If you made a mistake simply submit a new report. We will use the most current report in the system.
All health care entities licensed by the Colorado Department of Public Health and Environment are required to:
Keep track of the number of employees vaccinated against influenza.
Maintain influenza documentation for employees for a period of three years.
Annually submit electronic reports of influenza vaccination coverage.
Have 90% of staff vaccinated for influenza.

If a facility reports to the Centers for Medicare & Medicaid Services (CMS) or is required by statute to
report health care associated infections, influenza vaccination coverage should be reported through
the National Healthcare Safety Network (NHSN). All other facilities should report through the State
Health Facilities Portal. You only need to report in one system. Reporting to either system will meet
the state reporting requirement.
Any facility that reports through NHSN must be registered in NHSN, have added the Health Care Personnel Safety component, and joined the Colorado Influenza Reporting group (27648).
For total employee number, count employees who worked between October 1 – March 31.
To be counted, employees must work at least: One day.
People included in the count are employees, licensed independent practitioners, adult students, trainees, and volunteers.
Submit data through the NHSN portal by May 15.
