Downloading Survey Documents - Every survey will have at least one document posted by the Colorado Department of Public Health and Environment. Users should promptly download all attachments because:
-Attachments may not be available 30 days after the survey is finalized.
-Attachments include important information about next steps.
-Attachments include an official copy of the survey for user's records.
Uploading Revisit Documents - External users will only upload documentation when the Colorado Department of Public Health and Environment elects to complete a "Desk" revisit. Please note, external users will not be able to upload documentation until their plan of correction is in "Accepted" status.
