Using the Chrome browser, navigate to the COHFI home page: https://www.cohfi.colorado.gov/COHFI/Account/Login

Enter your Username

• If you have forgotten your username, follow the instructions here

Enter your Password

• If you have forgotten your password, follow the instructions here

Confirm and log in using the Log in button

If you are someone who needs access to an actively licensed or certified entity, you will need to find out who your COHFI Account Manager is. This is the individual that will need to create a new account for you and assign you rights within the COHFI system. If you cannot find someone within your agency to provide this information, please contact cohfi.support@state.co.us to ask CDPHE for this information. 

If the current COHFI Account Manager is no longer with the facility/agency please contact cdphe.healthfacilities@state.co.us to request that the COHFI Account Manager role be reassigned to a different user. 

• We will need the first name, last name, email address, and existing COHFI username (if one exists) of the individual you wish to be assigned the COHFI Account Manager role

If you are a new entity who wants to apply for licensure or certification, you will first need to submit a Letter of Intent (LOI). COHFI account access will follow.
