Initial applications can only be created and submitted by non-licensed entities. This process beings by submitting a Letter of Intent to allow the Department to create your COHFI access.
Licensed facilities/agencies have a one year license term. Each license requires an annual renewal application to be submitted. This is required to be submitted 60 days in advance of the license expiration date.
Update application allow currently licensed facilities/agencies to change their information, including Facility Name, Administrator, Director of Nursing, etc.
Changes of Ownership that meet the regulatory definition of a Change of Ownership as outlined in 6 CCR 1011-1 Chapter 2 Part 2.6 must file a Change of Ownership application.
