COHFI works best in up-to-date versions of Google Chrome or Microsoft Edge. Any version of Internet Explorer is not recommended.
Using older versions of browsers may not submit information. For instance, if you do not receive a successful submission pop-up when you click submit then your License Application, Occurrence Report, Plan of Correction, or Vaccine report was not successfully submitted. If you do not get a confirmation window, save if able, and try a different browser.
Due to changes in data validation, some Renewal Application sections will not allow completionsubmission without the assistance of Department staff. We are able to enter data for you if the fields are locked. Please email cohfi.support@state.co.us if you need us to enter missing data into any section of your Renewal Application.
A number of License Expiration Notifications were sent in error to facilities/agencies who had recently renewed their license, or completed a change of ownership. If you feel that you received a renewal reminder in error, you likely did. These erroneous notifications will be cleared from the system, and the system has been fixes to prevent this moving forward.
If you are unable to create an application, please read through the information on the Licensing Applications page of this User Manual. There are rules in place for when you can and cannot create certain applications.

As with any system, there may be some minor bugs experienced when updates are rolled out.
If an error message pops up, please take a screen shot and copy the URL address. Provide details about what happened and send the Division an email.
Please submit any issues to cdphe.hfoccur@state.co.us or cohfi.support@state.co.us and we will route them for review.
With some error messages - "loading content" the log in session for COHFI and/or an individual occurrence report might have timed out. Select and save as draft frequently when working on a report and/or be mindful of your time in COHFI. You will need to log back into a new facility session.
At times, there are duplicate entries of a victim, assailant or notes. Please note the finding and send us a message.
The OIT team has fixed the capacity load for data text limits, so the boxes should accept all copied texts.
HOWEVER, please follow these guidelines: DO not copy and paste from a word document with formatting in place. At this time, please use ctrl+shift+V to copy and paste without formatting
Please keep data responses relevant to the questions asked.
Message center notification working - three notifications to review messages about an Occurrence: external email, new message in 001 folder on the workbench and new message in the main message center in COHFI (envelope icon on main page).
Message center - please ensure to mark the messages as "read" for management of your 001 folder and message center.
Recommend checking the message tab after submitting a report to ensure the system indicates the department received the report.
When working through an event, the screen might act like a "pop up" window. You will not see a close button. Go ahead and X out of this screen.
Ensure to check -date fields and parameters when using the big list. Adjust the timelines accordingly for your specific search requests - ALL, 30 day, 90 day, this year, etc.
