-If all POCDs are marked "Ready" or if you have a POCD that cannot be marked "Ready" and you are unable to "Submit POC,", this may be a glitch. Send a COHFI note from the applicable survey's "Message" tab explaining the situation. Please allow one business day for a response.
We understand if a submission glitch may impair time POC submission. Sending a message timely ensures we know the system is responsible for the late submission. There is no need to call, email and communicate through multiple channels. Your COHFI message is sufficient.
Is your POC in Accepted Status? External users cannot upload documents unless they are in "accepted" status.
Are you trying to send files that exceed 25 MB? There is a file size limit. Break the submission into smaller sizes.
Sometimes the canned POC Accepted message does not populate with the documentation due date for your desk revisit materials. That information is on the Main POC Review Screen in a box called "Documentation Due."
In the event the POC/Revisit Coordinator did not enter a Documentation Due date, you would plan on submitting documentation seven days after the latest completion date of any POCD.
If you are unable to determine your documentation due date, please use the "Messages" button on the applicable survey to send a message to the assigned reviewer.
External users are encouraged to promptly access COHFI and review all surveys, messages and retrieve uploaded documents for safe storage on their systems.
At this time, all documents for any survey are deleted 30 days after the survey is finalized. A survey is finalized immediately if it is a 0 deficiency survey. Surveys with deficiencies are finalized upon completion and processing of the associated revisit.
You can check the "Survey Finalized Date" on the Main POC Review screen.
The elements of an acceptable POC are found in the "Packet" of information uploaded when the survey is released for your review. Please download the "Packet" from the "Documents" tab.
Click on the "Messages" button in the applicable survey BEFORE your due date and request an extension. Typically, a seven day extension is given upon timely request. Additional time is on a case-by-case basis in exceptional circumstances.
