The Facility Login page will be the first screen you see after successfully logging into COHFI. This grid will show you every facility/agency for which you have been assigned rights. The following information can be found in this grid:
Facility ID/License Number - These terms are interchangeable. This is the unique identifier for your facility/agency.
Facility Name - This is the name of your facility/agency.
Facility Type - This column identifies the type of facility/agency for that license or certification.
Facility Status -
Active: These are actively licensed or certified facilities/agencies.
Pending: These are typically facilities/agencies that are working through the initial application process.
Account Manager - This column will identify whether or not you are the COHFI Account Manager for each facility/agency. If there is a checkbox in this column, it means you are the COHFI Account Manager for that specific facility/agency.
Administrator - This column identifies the current Administrator of record. If you have submitted an application to update this information, it will not be updated until we actually process that application.
Licensing Type -
Licensed: These are licensed facilities/agencies
Sublicense: These are either Personal Care/Homemaker (HCPF Specialty Code 666) or In-Home Support Services ((HCPF Specialty Code 656). These are not licensed Facility IDs, however they will always be linked to a licensed Facility ID.
NotLicensed: These are facilities/agencies that are certified only, either through Medicare or Medicaid.
License Expiration - If a facility/agency is actively licensed, the expiration date for that license will be in this column
Address - This column identifies the current address of record. If you have submitted an application to update this information, it will not be updated until we actually process that application. Please note that facility-based licenses must go through the initial licensure process if they plan to change locations.

To log in to a specific facility/agency select the blue Log in button to the left of that facility/agency
The white box in the upper right side of the black menu bar will show you the facility/agency you currently have selected. All information shown will be specific to that facility/agency. The menu options you see will be determined by the access you have been granted for that specific facility/agency. If you do not see menu options you need, please contact your COHFI Account Manager to have your access updated.
The system may prompt you about renewal applications due, or renewal application in progress.
Selecting Cancel if you are not prepared to complete the Renewal Application is acceptable. This reminder will continue to pop up until the Renewal Application is submitted.
Depending on where you are in the renewal process, selecting Renew License will either take you directly to an existing in-process Renewal Application or it will take you to the prompt to actually create the Renewal Application.
After selecting a facility/agency, the main page you see will include an Announcements banner. This is the Department's method of communication anything you might need to know, any urgent issues, or any other information we feel is important for you to know.

To log out of one facility/agency and select a different facility/agency to work with select the Home menu option and select Facility Login from the dropdown menu. This will bring you back to the full Facility Login grid.
A second option users have available to them is the dropdown selection in the menu bar. This allows user to quickly move between various assigned facilities/agencies without having to return to the Facility Login screen.
