Once a license or registration document has been issued following the finalization of the initial, renewal, change of information, or CHOW application process, a license record will be created in COHFI. Licensing staff will generate a downloadable PDF license or registration document, which will be uploaded to the applicable license record in COHFI.
To locate current or historical licenses or registration certificates dating back to 2020 select the Licensing Menu, then select Licenses from the dropdown options.
Users can only see one facility or agency's licenses/registrations at a time.
Users must be logged into the specific facility or agency for which they want to view or download a license. The facility ID is shown in the red box on the left of the screenshot below.
Licenses or Registrations are listed in order of the date they were issued, as shown in the red box on the right of the screenshot below.
The type of license issued, the effective date, and the expiration date are shown on the grid within the green square.
In order to download any license, you will need to open the record for that license by selecting the Details button on the left, as shown in the green circle on the screenshot.
Users with a large number of licenses can search or filter their selections using either the search bar function, or the individual column filter search options to locate specific license documents.
For example, if the user wishes only to see renewal licenses issued, they can type "renewal" into the search bar, OR select the filter next to License Type and select either Equals or Contains and enter "renewal".
To download a license or registration document from the Licenses page:
Locate the record for which you'd like to download the license or registration certificate;
Select the blue Details button on the left side of that record, as shown in the yellow circle in the screenshot below;
Select the blue Download button to download the license or registration document, as shown in the green circle in the screenshot below;
The document will be saved to your downloads folder, or wherever your device is set up to save downloaded documents;
Print and display the license.
NOTE: Per 6 CCR 1011-1 Chapter 2, Part 2.9.2, The license shall be displayed in a conspicuous place readily visible to clients who enter at the address that appears on the license.
