When a facility/provider/agency is notified they plan of correction is accepted and desk revisit will be conducted by the Colorado Department of Public Health and Environment, external users will upload desk revisit materials by the documentation due date.
Uploading documents for desk revisits in COHFI for a desk revisit is one of the last steps of completing the survey process. First click on the "Documents" button to reveal the documents pop-up screen. The purple arrow in the figure below shows where to click to select the documents you want to upload from your computer. Once you have selected your documents, you will seen them display in the documents list, as depicted by the green arrow. The final step is to click the "Send Attachments" button as depicted by the red arrow. Users may need to repeat the upload process multiple times if there are many deficiencies that require desk revisit documents.
