Events requiring a final report will be located in category 003 on the workbench
Select arrow to open the category folder
Find the report that needs a final report
Select "details" to open the event to the landing/tracking page
Select final report on the landing page, which will open a new window with the report questions.
Select "edit" button first (found on left upper corner of report)
The final report appears in a new window.
Each reportable category contains questions specific to that type of report.
Select "Edit" button first to start entering information into the final report.
To begin placing information into the final report - select "edit" button first
Be mindful of time spent entering the information on the open page - 15 minute timeline in effect per session. It is recommended to select "save as draft" frequently.
Consumer (s) and alleged assailant (s) information - select "add" and a new window pops up for each area
Enter information (no edit button) and start typing in the required field information. Select "Save" button and window closes back to final report page.
Multiple victims can be entered into a final report. Select "add" again to enter another consumer and save.
Multiple assailants can be entered into a final report. Select "add" again to enter another consumer and save.
After completing and consumer and/or assailant information, identifying information will show up in the appropriate grid on the final report.
Each reportable category contains questions specific to that type of report.
Final report questions are a combination of text entry, drop down boxes, or check boxes.
Continue filling out all fields and sections of the final report.
Police notification is required by statute on all reportable abuse events.
Save as Draft: Final report information can be saved prior to completing the final report. Save as Draft will save all information that has been entered, but will not submit the final report to the department.
Save & Submit: When the final report information is complete, Save & Submit will submit the initial report to the department.
Validation errors: If required fields are missing data, a pop up message will appear.
If save/submit is successful, a pop up message will indicate a successful submission of the report.
Check message center - auto populates a note: "Final report submitted" that time stamped.
Cancel/Delete: Select either Cancel, or Delete to exit the final report. Information will not save.
Close: closes the page, ensure action is taken to either save as draft or save & submit to prevent instances of data loss.
If all the required fields are filled out, the final report will be successfully submitted to the department.
If there is missing information, a validation error will pop up to direct the user on what information is required. Select "edit" again and complete the report by selecting save & submit option again.
After the submission of a successful final report, a note will generate.
Check message center within the specific event to view the created system note - final report submitted with the date/time.
The occurrence report moves to category 004 "state review"
Investigators will review the report and ask questions as needed about the case.
Questions will be sent within the specific case ID
Providers will receive an email alert to check messages. Navigate to workbench 001 to the specific case and select messages once the report is opened.
