Report Health Care Workers (HCW) who have been COVID-19 vaccinated at your facility twice a month.
Sign into COHFI.
Select your facility.
Select Vaccination Reporting and then COVID Vaccination Reporting.
Complete the form.
Select Submit Vaccination Report.
Select Vaccination Reporting from the top navigation bar and then select COVID Vaccination Reporting.
If you do not have Vaccination Reporting available then check with your facility/agency's account manager to allow you access.
Complete the form by answering each question with the number of Employees, Direct Contractors, and Support Staff.
The reporting is due on the 1st and 15th of every month.
Select Submit Vaccination Report to submit your report.
If you made a mistake simply submit a new report. We will use the most current report in the system.
When you have to report next, you can select the Load Previous Submission button to load the last submitted report. Then you can update the form with your current numbers, or if your numbers have not changed you can select the Submit Vaccination Report button.
Load Previous Submission does not load the previous submissions of the question on staffing resignations. Those answers should be updated for every cycle.
A worker subject to the rule is an employee, direct contractor, and support staff. They are defined as an individual who has the potential for exposure to clients of the facility or agency and/or to infectious materials, including bodily substances, contaminated medical supplies and equipment, contaminated environmental surfaces, or contaminated air.
These individuals may include, but are not limited to: licensed independent practitioners; students and trainees; individuals who directly contract with the facility or agency to provide services, whether on a permanent or temporary basis; visiting nursing staff; individuals who are affiliated with the facility or agency, but do not receive wages or other remuneration from the facility or agency; and persons not directly involved in client care but are potentially exposed to infectious agents that can be transmitted to and from the individual providing services and clients of the facility or agency.
Examples of workers who are not subject to the rule include but are not limited to: billing staff who work from home only, staff who work at an offsite office space that patients and clients do not have access to, mental health practitioners who provide telehealth services only and do not have an office space that patients and clients can access.
In general, people are considered fully vaccinated:
2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines,
OR
2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine
If you don’t meet these requirements, regardless of your age, you are NOT fully vaccinated.
People who have received any amount of vaccination but
are less than 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine,
OR
are less than 2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines.
The reporting cycle is the first of the month through 11:59 pm on the 14th of the month and the 15th of the month through 11:59 pm on the last day of the month. All reports made during those cycles will be used for that cycle.
If you made a mistake on your submission all you need to do to update your numbers during a cycle is to submit another report. We use the most recent report in a cycle for the report. For this reason, early submissions are not accepted. Early submissions are a correction of the current cycle's submission.
