Spinal Screenings

Our campus will be conducting spinal screening of all female fifth grade students. 

State law requires that schools must screen students for abnormal spinal curvature in accordance to the following schedule:

• Girls will be screened two times, once at 10 (Fifth Grade) and again at age 12 (Seventh Grade)

• Boys will be screened one time at age 13 or 14  (Eighth Grade)

Nurse Brown will check your child for signs of spinal problems like scoliosis. 

Catching a spinal problem early can make the treatment much easier. Not treating spinal problems can lead to serious health problems. 

The screening is simple. Nurse Brown will look at your child's back while she stands and bends forward. 

Important Recommendations:

• Students should bring shorts to school for the exam. 

• Girls should wear a thin t-shirt or a sports bra underneath their shirt on the day of the exam. 

• If girls are wearing a sports bra, it is recommended they remove their shirt for the screening. 

The school will send you a letter if your child does not pass the screening. The letter will tell you how to follow up with a doctor. This screening is not a medical exam. Your child still needs to see a doctor for checkups. 

If you do not wish to have your child screen for religious reasons, you must submit an exemptions to the school no later than April 23.

Human Health Education and Instruction Parental Consent Form

As part of the Human Health Education Texas Essential Knowledge and Skills (TEKS), the Life School Board has approved the following puberty education videos, titled Always Changing and Growing Up, to be shown to our 5th grade students. The Texas Education Code now requires parents to Opt-in for their child to watch these videos (rather than Opt-out).

As a parent you have the option to view these videos at home with your child rather than completing the Opt-in form and approving your child to see these videos at school.

If you would like to view the videos at home or to not view them at all, then no further action is required on your part.

If you wish for your child to participate in a viewing of these videos at school, then you must complete and return the linked Opt-in form.

LSC - Parent / Guardian Request Center 

Per the Life School Handbook, parents are required to inform the school on any important changes such as a new phone number, home address, email address, etc. Parents are also required to submit a written request for their students records. Please fill out the form in its entirety. Mr. Perry will then call to verify the request. Once verified, the request will get fulfilled within 24-48 business hours. Contact Mr. Perry with any questions.
