When NASA used the metric system and Lockheed Marten used the imperial system when building satellites
. In 1999 a group of Lockheed Martin engineers used the imperial system for the Mars orbiter while the rest of the team used the metric system. This mishap cost them 125 million dollars. When they sent it up the spacecraft was off and was lost to space.

The safer inspector forgot to replace a valve at the Piper Bravo oil rig.Â
In July 1988, oil workers were evacuated from the oil rig after an explosion. The blast killed 167 of the 226 men working there during a routine check inspectors removed and put back safety valves. All except one which was never put back on the valve. A worker unknowing that the valve was missing pushed the start button and gas began to leak out and explode. The money lost in 1994 would have been 3.4 billion dollars; nowadays it would have cost 5.1 billion dollars.

In January 1986 the Challenger a NASA spaceship, exploded 72 seconds into its flight; everybody on board died. The explosion was blamed on NASA for the faulty equipment. This cost 11.1 billion today
