Your Physical and Immunization Records are your medical documents and proof of vaccinations against infectious diseases like Measles, Mumps, Rubella, and more.

Most campuses across the United States require students' immunization records given that students can register for in-person courses and will be exposed to the presence of other students and faculty in the campus setting.

Most SUNY, CUNY and private colleges require that students submit their Immunization Records by completing a campus-specific form, and emailing it to their Admissions Office or Health and Wellness Center, or uploading these documents to your student portal (like CUNYFirst if you are a CUNY student).
