Faculty and staff are encouraged to support students by completing a Student CARE Referral when someone may be experiencing challenges that affect their ability to succeed. CARE focuses on the whole student and allows coordinated outreach and non-clinical case management. For students experiencing academic difficulties, you are encouraged to reach out to students experiencing academic difficulties prior to sending a CARE referral to Student Services.
Submit a CARE Referral for any of the following reasons:
Academic struggles or sudden performance changes
Poor attendance or disengagement (in person or online)
Missed assignments or lack of participation
Changes in behavior, motivation, or communication
Personal, financial, mental health, or basic needs concerns
Students not responding to outreach
The form is available in Brightspace under More Tools or via this CARE Referral link. Note that the former Early Alert form is no longer in use.
