Self Assessment -Purpose
Annual GHC conducts an annual program self-assessment between February and the end of March. Results must be submitted to the DFSS/Chicago Commons planning team and the agency’s monitoring team using the Self-Assessment Report Template and the Self-Assessment Summary Tool (MIR). HS/EHS/CCP funded agencies must have the final report and action plan approved by the governing body and policy committee and share results with staff and parents in the CSD department.
What is the Community needs Assessment?
Gads Hill Center and Partners are required to conducts a community needs assessment to collect data about community strengths, needs, and resources every five years. Agencies will use this data to make decisions about long- and short-term program goals, selection criteria, the type of services provided for children and families, and to determine collaboration possibilities with other agencies. Community needs assessment data will be updated each year as needed.
