You MUST:

1. Type your name and date
2. Validate the form
3. Print it, and
4. Sign it *BY HAND* next to your printed name.

Your application package will be returned to you if you fail to sign this paper by hand.

IMPORTANT: Signatures must NOT be older than 90 days when IRCC receive your application package.
