Additional Family Information

IMM 5406 Form is considered COMPLETE if:

Section A:

• form contains Names of requested family members, their date of birth and place of birth. If the mother and/or father are deceased, that should be reflected on the form (note that "unknown" is also accepted; the fields just cannot be left blank). If you are unsure, write 'Letter of Explanation'and explain details in there.
• PA & SPR's full present address is written (required); however, for mother & father, only city & country is also acceptable, unless they reside in USA, in which case, full address is required.
• If PA lists their full present address & under SPR they write "same as above", it is also acceptable.

Section B:

• dependents listed in Form IMM0008, are also mentioned in this section. (if address information for the dependants is missing or incomplete, form will still be accepted).

Section C:

• some fields are missing or incomplete in this section (form will still be accepted).

Section D:

• The form must be signed by the PA. (Not Sponsor)

Application Returned if:

• IMM 5406 form is not provided
• required fields are incomplete.
• The form is not signed. (Dependent Child (FC3) must complete and sign form regardless of age. If under 18, must be signed by a parent or legal guardian.)

[EXCEPTION]:

• Application NOT Returned if any information is missing for deceased family members.
• Application NOT Returned if step-children are missing or have incomplete fields.
• Application NOT Returned if full addresses are not provided for applicants, but this information can be found on other IMM forms.

Should Sponsor Fill "Additional Family Information" - IMM 5406 Form?

NO, but you do have to include your SPONSOR Spouse in your IMM 5406 Form.

Some forms are generic and are used for other immigration streams where couples are immigrating together. That's why [N/A] boxes are placed there. If both Sponsor and Principal Applicant's information are required in forms, they would place that form under "BOTH SPONSOR & PRINCIPAL APPLICANT".

In Document Checklist IMM 5533, IMM 5406 are ONLY included under PA's forms list, not Both. Therefore, it must ONLY be filled by PA (and DEP's, if any) and only PA's (and DEP's, if any) information is required.

For Spousal Sponsorship purpose, Spouse is primarily considered as "SPONSOR".

Principal Applicant's forms must ONLY include those who are also accompanying him/her as DEPENDANT (not as SPONSOR). For application purposes, your Canadian citizen/PR spouse is not your "DEPENDENT SPOUSE", they are only the SPONSOR and they do not fill out any PA forms. These forms are generic for all streams of immigration, so they are not referring to Canadian spousal sponsors, they are referring to spouses of applicants applying through another stream (parent, skilled worker, EE, etc) where spouse act as a DEPENDANT rather than PRINCIPAL APPLICANT.

If you still have desire/urge to fill your Canadian Sponsor's details on PA's supplementary forms, there's no harm in doing that. However, No file was refused/returned in both cases.
