Medical Examination Request is sent by IRCC to Principal Applicant AFTER Sponsor is approved and "Decision Made" is displayed on Sponsor's column in ECAS. It can either be sent by EMAIL, GCKey or by Post.
You are NOT required to give your medical examination upfront.
Medical Exams are valid for 12 months from their "Medical Passed" Date.
You can’t get an upfront medical exam if you’re being sponsored as a spouse, partner or child. Wait for our instructions before going for your medical exam.
You must request IRCC for Medical Exam Letter (MEL) for your newborn child. You must also add your non-Canadian child/newborn in your Spousal Sponsorship application.

Medical examination reports are evaluated by CHC New Delhi. You may receive a re-medical request from them. You should add the domain “international.gc.ca” and "cic.gc.ca" to your email safe list. As there have been cases where email requests have been delivered to the recipient’s junk mail folder. Hotmail is particularly notorious.
You only need Polio vaccine when flying out of Pakistan, not for this medical examination. Visit any government hospital with a photocopy of applicant's passport. Contact to reception and ask for polio vaccination and polio certificate , within 30 minutes applicant will be done.
From Center for Disease Control (USA):
Although no adverse effects of IPV have been documented among pregnant women or their fetuses, vaccination of pregnant women should be avoided on theoretical grounds. However, if a pregnant woman is at increased risk for infection and requires immediate protection against polio, IPV can be administered in accordance with the recommended schedules for adults.
The Panel Physician sends the completed immigration medical examination (IMM 1017E, IMM 5419E and other reports) to the appropriate CIC regional medical office. The CIC medical officer will conduct a review and assessment of the files. This process is referred to as an immigration medical assessment. Upon completion of this assessment, the regional medical office enters the result of these assessments in GCMS, which in turn notifies the responsible visa/immigration office.
After a medical examination by a Panel Physician, the results are sent for review to the CIC medical office in New Delhi. The CIC medical office will assess the results and send their assessment to London visa office. Processing times can vary but generally for permanent resident applications, it normally takes three months after the medical office receives the medical examination results from the Panel Physician.
