As per your email request, you have been scheduled for Canadian Health Assessment against date 15^th October 2018 at 09:00 AM at IOM sub office Lahore.

Please find the attached Client Declaration and Notice (eMedical) which you must read before visiting the IOM clinic for your appointment

Required documents to bring:

1. Valid Original passport along with 2 photocopies of first two pages of passport.

(If original passport is with CIC, please bring original CNIC (for adults) and Family Relationship Certificate (FRC) issued by NADRA (for children) along with two photocopies of the passport.

2. Medical Request Letter/ Acknowledgement Letter from Canadian High Commission (in case of upfront medical examination no form or letter required)

3. Vision card or any previous medical history record (if have any).

4. Total Fee for the above mentioned is PKR. 13, 000/-it should be in cash.

(Screening at Lahore center applies 16% GST on top of the above service fee)

5. Fasting state is not required for the screening.

Health Screening is comprises of following and according to visa class & age.

Blood Sampling (If required according to visa class)

General physical examination.

Chest X-ray according to age

Urine examination according to age

General Instructions:

For children 5 to 11 years

They have to bring the result sheet of the last annual examination.

For elderly

Applicants have to mention about their mobility need.

For all ages

For female

It is advisable to all female applicants that not to proceed health screening during their monthly period. In case of your monthly periods, please get an appointment date on any later date

Note: Do not bring any kind of luggage and unnecessary belongings with you.

“In normal circumstance, the minimal health screening time vary from 2 to 3 hrs however the exact time can be confirmed by the registration staff at the concern medical office at the time of your appointment”

Parking facility is not available at IOM Clinics.

• Completed MEDICAL REPORT CHEST X-RAY REQUISITION AND REPORT form (Page 5 and 6 of IMM5419) (in PDF)
• Duly completed Re-assessment Form (in PDF/word)

Please visit this website: http://www.cic.gc.ca/dmp-md/medical.aspx for identifying Panel Physician (PP) in your vicinity and take this email to the Panel Physician (PP) for the re-assessment(s) to be arranged. For clients outside India, please remember to advise the PP that the results will need to be sent to India.

You have 7 days from the date of this email to contact PP and comply with this request. Failure to do so may result in the closing of your Immigration medical file, which may result in the refusal of your application.
