The General Student Information section is the first section you see when creating or opening an IEP.
To assist with clarity, the "eSER Menu" dropdown at the top of this page is in the same order as the eSER website.
This section contains tabs for:
Basic Information
Evaluation
Participants
General Student Information
Special Factors
Navigation
IEP > IEP List > (choose IEP) > IEP Section > General Student Information
Basic Information
The gray fields are automatically populated based on eScholar records and the type of IEP that was created.
Type of IEP
Team Meeting Date
Current Grade
Home Based School
Other School Location
To edit these fields, click the Check Out button. The dropdowns then offer these options:
Type of IEP
Category
Regular
Gifted/Talented
Services Plan
Type
Interim
Initial
Review
Home Based School and Other School Location
The options shown will be different for each user since they are based on the student's jurisdiction.
The checkboxes for Age of Majority and Education Rights of Children with Disabilities do not have to be chosen for the IEP to be saved at this point.
Click Save to exit this section, or click on the next tab. If you are finished editing, click Save, then Check In to exit out of this section.
However, if you intend to edit or make changes in the other tabs in this subsection, you can stay checked out until all these tabs are completed. Then click Check In when finished.
If you make changes but do not click Save, then attempt to Check Out, the system will produce an Unsaved Changes popup to confirm that you do not want to save any changed data. Clicking Yes will Check In and return all sections to their previous state.
Click the Evaluation tab, to the right of the Basic Information tab, to continue.
Evaluation
The Evaluation or Waiver Date field should be edited via the Evaluation section. From there, it will populate automatically in this Evaluation tab.
This field can be changed later using the "Update Evaluation" feature. (See below: Visual Guides > Update Evaluation.)
"Evaluation or Waiver Date" is also labeled as "Report Disseminated Date" in different sections of eSER. (See below: Visual Guides > Similarities.)
In the Evaluation table, the Exceptionality, Primary Exceptionality, and Exceptionality Detail columns are populated based on what is entered in the Evaluation section.
Participants
The IEP Participants table shows columns for IEP Member and Participant Name.
If not already checked out, click the Check Out button to add or edit participants. This will produce a "+" button, which can be clicked to add a participant.
The Add/Edit Participant popup will appear with this dropdown:

General Student Information
There are two questions regarding verbal skills and impairments at the top of this subsection. These must be completed. Click in the desired bubble, and then click the gray Save button.
The section must be checked out for the gray Save button to appear and edits to be made.
In the text boxes, narratives can be added regarding the student's strengths, evaluation results and the like.
The small box at the bottom right of each text box contains three tiny diagonal lines. Clicking and holding on these lines allows the box to be dragged to a larger size. This modification may make it easier when typing longer narratives.
Text boxes throughout eSER have limitations on word count. Depending on the section or text box, the limit is between 2,000 and 8,000 words. If you experience problems, please contact system support.
The narratives can be edited by clicking the pencil icon at the right of each text box. Enter the narrative in the resulting popup, then click Save to commit and return to the subsection.
Click Check In when finished, or go to the next tab.
Special Factors
Similar to General Student Information, this subsection allows for narrative about further student data, including behaviors, communication and health needs, and the like.
The Transition Courses of Study - Attach plan to IEP dropdown offers these options:
Individual
Prescription for Instruction
Individual Graduation Plan
Individual Graduation Plan for LEAP Connect Students
Educational/Career Plan for LEAP Connect Students
The Educational Needs boxes correlate to the next subsection, Instructional Plan. Unless these boxes are checked, those tabs will not appear in Instructional Plan.

Click Save to commit.
Click Check In if finished. If not, then stay checked out, and go to the next dropdown item, Instructional Plan.
These are samples of the five tabs and subsections that may be seen.

The Update Evaluation button can be used to chose a different Evaluation or Waiver Date in the IEP Evaluation tab. This button will not appear if the IEP is an Initial IEP.
If an Initial IEP's evaluation needs to be changed, change it to a Review IEP by going to Basic Information > Type of IEP > Type dropdown. Update the evaluation using the process shown here, and then change the IEP back to an Initial IEP.
Click Check Out.
Click the Update Evaluation button.
If there is no Update Evaluation button shown at this point, and the date here is not valid, it means there is no valid evaluation available under the Evaluation section. You need to create a new Evaluation with a valid date. This will allow the button to appear.
Choose a date from the dropdown and click Save Changes.

Infant 0-2 / Prek 3-5
If the student is shown as Infant 0-2 or PreK 3-5 in eSER, check the grade in eScholar. eSER does not recognize codes for PreK 3 or PreK 4. It only recognizes Infant 0-2 (Code 15) and SPED 3-5 (Code 20). Below is an example screenshot of grade placement data in eScholar:
Wrong grade
The grade shown in the Student Profile "Grade" field is the grade that will be saved on the IEP once it has been submitted. It can be still changed as long as the IEP is in draft status, by changing the grade in the dropdown and then clicking the Save button. After the IEP is submitted, the grade only be changed via a request to eSER system support.
