Referral Activity

1. Click the "+" button to enter a new activity.

2. This will navigate to the Referral Activity View/Edit page.

3. The following fields are available:

   • Immediate Referral Reason

     1. Violent Behavior

     2. Severe or Low-Incidence Impairment

     3. Out-of-State Transfer

     4. Infant/Toddler

     5. Previous Special Education Student

   • Grade at Referral

     1. Not Provided

     2. 1st

     3. 2nd

     4. 3rd

     5. 4th

     6. 5th

     7. 6th

     8. 7th

     9. 8th

     10. T9th - Transitional Ninth

     11. 9th

     12. 10th

     13. 11th

     14. 12th

     15. Infant Program (0-2)

     16. Preschool Program (3-5)

     17. Kindergarten

     18. Not Enrolled

     19. Unavailable Non-graded

   • Referral Reasons

   • SBLC Entry Date 

     1. SBLC = School Building Level Committee

     2. This field refers to the date the meeting was held.

   • SBLC Decision

     1. No further action at this time

     2. Regular Education with Support Services

     3. Individual Evaluation

     4. No Services Required (Infant)

     5. Pupil Appraisal Interventions

     6. Section 504 Eligibility Evaluation 

     7. Interventions Through RTI Process

     8. Pupil Appraisal Support Services

   • SBLC Decision Date

     1. This field refers to the final date a decision was made.

   • Surrogate Parent Needed

   • Surrogate Parent Assigned Date

   • Surrogate Parent End Date

   • Part C Transition

     1. This refers to early childhood students transitioning from Early Steps to Medicaid Part B as they turn 3 years old.

   • Transition Meeting Notice Received Date

   • Transition Meeting Attended Date

4. Click Save to commit the data.

5. Click Return to Summary to navigate back to Referral and Screening Information.

Student Screenings

1. Click the "+" button to enter a new activity.

2. The Save Screening popup will appear., with three fields:

   • Screening Type

     1. Sensory Processing Screening

     2. Social/Emotional/Behavioral Screening

     3. Educational Screening

     4. Hearing Screening

     5. Vision Screening

     6. Health Screening

     7. Speech/Language Screening

     8. Motor Screening

     9. Assistive Technology Screening

   • Screening Date

   • Screening Result

     1. Advanced

     2. At Risk

     3. Normal

3. Choose the desired options from the dropdowns and enter a date, then click Save to commit the data.

4. The new screening will appear in the Student Screening table.
