7th Grade Immunization
Parents
The Tennessee Department of Education requires that all schools comply with the requirement of T.C.A. §49-6-5001 that each child in the school be vaccinated against disease. The Collegiate School of Memphis is complying to and following Tennessee state government regulations but also wants to ensure the safety and well-being of all students and staff of Collegiate.
7th grade students at Collegiate:
The requirement of the Tennessee Department of Health states that all students entering 7th grade must provide proof of:
Hepatitis B vaccine
Tetanus-diphtheria-pertussis booster ("Tdap")
Verification of immunity to varicella (2 doses or history of disease)
Proof of vaccinations on the Tennessee Department of Health CERTIFICATE OF IMMUNIZATION form is required for all 7 graders, and the Complete 7th Grade or Higher box must be checked. This form is available from your student's personal physician or from the health department.
Students not in compliance/who have not provided the indicated immunization documentation by Friday, October 4, 2024, will not be allowed to attend school-physically or virtually - and all computer accounts will be suspended until the form is provided. Any school days missed due to non-compliance will be unexcused.
If you have any questions regarding the completeness of your student's immunization record, please contact Collegiate at 901.591.8200. Thank you in advance for your immediate assistance and compliance to this request and for helping us ensure a safe and healthy environment for all Collegiate scholars and staff.
Sincerely
Warren M. Grimm, Principal

Fresh Friday
National School Lunch Program - Community Eligibility Provision (CEP)
The Collegiate School of Memphis today announced its policy for free and reduced-price meals for children served in schools under the National School Lunch Program and/or School Breakfast Program. For School Year 2024-2025 will continue participation in the Community Eligibility Provision (CEP). No further action is required of you.
This USDA provision allows your child(ren) to participate in the school meal programs without having to pay a fee or submit a meal application.CEP is a key provision of the Healthy, Hunger Free Kids Act of 2010, which allows schools and school districts in low-income areas to eliminate school meal applications and serve breakfast and lunch at no charge to all enrolled students.
CEP gives food service professionals more time to focus on preparing nutritious meals that their students will enjoy and gives students more time to eat those meals by cutting down on time spent in the lunch line. More importantly, by offering all students a nutritious breakfast and lunch at no cost, CEP helps ensure more students come to class well-nourished and ready to learn.
Children need healthy meals to learn. The Collegiate School of Memphis offers healthy meals every school day. Breakfast and lunch meals will follow the United States Department of Agriculture (USDA) guidelines for healthy school meals, so please encourage students to participate. “In the operation of child feeding programs, no child will be discriminated against because of race, sex, color, national origin, age or disability.”
Parents or guardians who need further information may contact Mr. Buchannan, Child Nutrition Program Director at jbuchannan@collegiatememphis.org or 901.591.8200.

Summer Reading Lists
Click here for more information about summer reading at CSM and to access the summer reading books.
From the CSM English Department:
This summer, read as many books as you can. Read books you enjoy, and read a lot of them.
From all those books you read, you are asked to choose one or two books (depending on your grade level) to tell your teacher about once class begins in August. For this assignment, choose books that are driven more by words than by pictures—not graphic novels or manga.
You will not have to write about the texts or answer questions about the texts over the summer; however, you will be evaluated on your knowledge of both books during the first week of school. Be prepared to discuss characters, plot, and key details from each book the first week back!
Click here for a list of books that students have loved in the past broken down by grade level. Although there’s a good chance you will enjoy these books, too, you are not required to choose from this list.
If you are unfamiliar with the text, feel free to look up a summary of the book on Goodreads.com or Amazon.com.
Final Report Cards
Final Report cards will be mailed home on June 11, 2024. If you have moved this school year, or would like to request a change of address, please email CSM Registrar, Ms. Williams (awilliams@collegiatememphis.org) with your new address. You will also be able to view your child's final report card on the MySchoolApp website on June 11, 2024 beginning at 2:00 p.m. Students who have a hold on their account will not receive their report card or be able to view their final report card online.
