Requirements:  All Part 3 Summons require a case file

• General Report

• Officer Notes

• Part 3 Summons - sworn to and BDL as POA Summons

• Call records to start a POA E-Tracker

• Vehicle CPIC -  usually completed by Records

• CPIC Dr Lic and Paris 1&2 (Driver's Licence and Convictions) - usually completed by Records or CPU

When applicable:

• MVC Report -CROMS Report

• Witness Statement

• Youth POA matters must be scheduled on FRIDAY only, 4 weeks from date issued at 45 Main St E, City of Hamilton, Rm 300, at 13:30hrs
